Most of the authers prefer high ligature varicocelectomy while others consider the scrotal one is a complete operation, this attracted our attention to do this study. Forty six infertile males with clinically apparant varicocele were devided into two equal groups, the first group were surgically corrected by scrotal approach while the second by high ligature operation [H.L.L]. A follow up every 3 months for one year to evaluate the post operative results showed that, there is improvement in all semen parameters of both groups. As regards the occurace of pregnancy there are 10 out of 23 in H.L.O. compared with 7 out of 23 in the scrotal approach
